How you'll make a difference

Our social work assistants make a real difference. Every day they work with families to help children and young people who have a significant disability or impairment and have difficulty achieving their ambitions.
  • Listening to young people with a disability and putting them at the heart of all that we do.
  • Working with young people in South Gloucestershire who need our support with their transition pathway into adulthood.  You will work with young people and others involved in their care to help plan goals for the future, to develop their skills and confidence to help them live as independent a life as possible.
What will you be doing?  
  • You will be working with young adults who require social care services, and need support to help plan for the next stage in their life this will involve assessing and reviewing cases, care plans or reablement programmes, making recommendations for changes or adjustments as necessary.
  • You will be working in a strength based conversational model of practice (3 Conversations) and working in close partnership with our Preparing for Adulthood team.
  • It will be key for you to work and liasise with the community, volunteers and external contacts in order to gain wider and more inclusive support for clients.
  • Working alongside our Social Workers, you will help families understand that their young people are now adults and are able to make their own choices for greater independence.  Navigating the transition process into adulthood with different professionals and legislation involved. 

What we need from you

  • A relevant qualification (NVQ Level 3 in health or social care) or experience of statutory or voluntary sector work in the health or social care field.
  • You’ll have the ability to gather information from a variety of sources and follow assessment processes, maintaining timely and accurate records.
  • You’ll understand the needs of service users and have good interpersonal skills with the ability to work alongside service users, other professionals and service providers in a way that promotes independence and personal choice. 
Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
